I confirm the information I have provided is accurate. I understand that FightCity is not medically trained. If I have any health concerns, it is my responsibility to seek professional medical advice before using the gym or attending classes. To the best of my knowledge, I have no medical condition that would put me at risk when training at FightCity. I understand that I am responsible for familiarising myself with the equipment and how to use it safely. If I’m unsure, I will ask a coach or member of staff before attempting to use it. If I choose not to, I accept that I do so at my own risk. I agree to pay my membership fee by direct debit, in advance, by the last working day of each month. I understand that late payment may result in my access being restricted. Repeated missed payments may result in my membership being terminated. I agree to maintain a safe and tidy training space at all times. I will clean down equipment after use and follow any hygiene or distancing protocols in place. I understand that my key fob is for my personal use only. Letting non-members into the gym is a breach of membership terms. If I’m found doing so, my membership will be cancelled with immediate effect and without refund. Reinstatement will only be considered if any loss is reimbursed in full. I accept full responsibility for any injury, loss or damage I may experience while using FightCity facilities. I waive all claims against FightCity, its owners, coaches, and staff for any injury or damages incurred. By signing below, I confirm that I understand and agree to comply with all FightCity membership rules, including those related to access, equipment use, behaviour, and gym policies. I understand that failure to do so may result in immediate cancellation of my membership without refund or notice.*
